BlepharitisBlepharitis – General InformationBlepharitis is considered to be an inflammation of the eye lids. This type of condition is characterized by the inflammation of the eyelid margins. The disease, in most of the cases, causes symptoms such as redness of the eyes or itching and irritation of the eye lids, usually in both eyes, with some rare cases in which only one eye is affected. Its occurrence is in almost all cases confused with another eye disorder known as conjunctivitis and because of its recurring character it is the most encountered source of recurrent type conjunctivitis, especially in older patients. This condition is in most of the cases treated as dry eye disorder by patients because of the gritty sensation it can cause to the eyes even the lubricating treatment drops don’t bring any improvement this condition. This kind of condition is classified by the specialists in two types such as Anterior Blepharitis, in which it affects the front area of the eye lids, localized in the surrounding of the eye lashes. The sources of these affections are the seborrheic dermatitis, dandruff alike, and the rare infection with Staphylococcus bacteria. The second type of the disease is considered to be the Posterior Blepharitis and affects the back of the eyelids, the area that gets in contact with the eyes. This type of the disease is provoked by the oil glands existing in that region, and is the most encountered form of the eye disorder. This disease is usually one part of the area of seborrheic which also affects the scalp, lashes, eyebrows, nasolabial folds or ears.Blepharitis – SymptomsDepending of which type of Blepharitis the patient suffers, he will probably experience symptoms such as eye irritation, burning, tearing, foreign body sensation, crusty debris, are taking place usually in the lashes or in the corner of the eyes and, especially on the lids, and also dryness, and red eyelid margins. It is very important so seek medical attention as soon as possible the patient noticed the symptoms. If the Blepharitis is caused by bacteria, the long term symptoms are thickened lid margins, dilated and visible capillaries, trichiasis, eyelash loss, ectropion or entropion. The significant erosion of the cornea can also be considered a symptom, taking place in the third layer of the lids. The eyelids can become greasy and even crusted progressing all the way, getting to affect the lashes. This can make eyelids stick together during the night. A patient does not have to worry if he has to get his eyes open in the morning due to these kinds of sticky secretions. In some cases, the patient can experience just the presence of dried tear secretions in the beginning of the day that can be sensed similar to small grains of sand. In this case, the doctor will be capable to diagnose Blepharitis after a carefully and safely exam of the eyes and eyelids. The doctor usually uses a special magnifying instrument to perform the examination. In some cases, a sample of the skin surrounding the eyes can be taken to see if bacteria are present.Blepharitis – TreatmentActually, the disease is very hard to cure and topical treatment is insignificantly effective. The most important treatment is the self care treatment a daily trial of lid margin hygiene. This way to treat Blepharitis needs to be done correctly so it bring improvement and avoid the relapses, and consists in a trial of 3 stages. The first stage is done by softening of the lid debris by applying a warm wet compress on the lids, usually a wash cloth, and keeping it there for more than two minutes. The second stage is a mechanical removal of the lid margin debris, by washing the surface of the skin with a wash cloth; in most of the cases is used a facial soap or baby shampoo diluted in water, which must be a repeatedly and gently action. The third stage is done using antibiotic to reduce the lid margin of the bacteria and consist in a process consisting in, first cleaning and then applying a small amount of antibiotic prescribed by the doctor, usually an ophthalmic ointment, using the finger when the eyes are closed. This type of treatment is used usually at bed time to avoid any blurry vision. There is a guide that is considered very common in the treatment of Blepharitis, and is more challenging to be done by visually affected and frail patients and it requires almost perfect motor skills and, generally a mirror. The first phase of the process consists in applying a wet compress on the eyes for more than five minutes twice a day. After this, using a mirror, the patient must use a Q- tip soaked in a baby shampoo, and rub along the lid margins, then tilting the lid with the hand. The third phases consist in applying the antibiotic, usually erythromycin, in the eye and then pull the lid away from the eye with the hand. The glands located on the lid margin are the meibomian glands, the zeiss glands, and the moll glands, are the target of the massage. Depending on the stage of the inflammation on the lid margin, there are a combination antibiotics or steroid drops and ointments which are prescribed to lead to an instant improvement. Due to this fact, this increases the risks of highly intraocular pressure and back subcapsular cataract formation. Because the cataract formation is an irreversible process and the intraocular hypertension can occur, increasing the risk of glaucoma appearance causing permanent visual loss and both have to be checked each month. These steroid induced cataracts or ocular hypertension can appear in patients of all ages. Doctors usually treat Blepharitis in the same way they treat seborrheic dermatitis and consist in a careful topical anti-inflammatory medication such as sulfacetamide or short trials of mild topical steroid medication. Drugs like anti-fungal treatment such as ketoconazole prescribed as Nizoral are recommended for seborrheic dermatitis, and most of the doctors usually don’t prescribe anti-fungals for seborrheic Blepharitis. If the topical treatments for Blepharitis do not bring any improvement, patients must perform an allergy test. Drugs like Patanol, Optivar, Elestat, and other optical antihistamines like Zaditor are effective in patient with allergic inflammation. |
