BlepharospasmBlepharospasm – General InformationBlepharospasm is considered to be an abnormal and involuntary blinking called spasm of the eyelids. Blepharo means eyelid and spasm means uncontrolled muscle contraction. The term Blepharospasm can be related to any abnormal blinking or eyelid tic or twitch resulting from any cause, progressing from dry eyes to the Tourette's syndrome, leading also to tardive dyskinesia. This type of the disease is usually called Benign Essential Blepharospasm to make the difference between the less chronic secondary blinking disorders. The benign term shows the condition, which is not life threatening and the essential term has a medical meaning, indicating the unknown cause of the disease. It is considered to be a both cranial or a focal dystonia. The cranial term is related to the head and the focal term shows confinement to one part. Patients suffering from this kind of disease have normal eyes but the visual disturbance is because of the intentional closure of the eyelids. The disease, in most cases, causes symptoms such as redness of the eyes or itching and irritation of the eye lids, usually in both eyes, with cases in which only one eye is affected. Its occurrence is in almost all cases confused with another type of eye disorder known as conjunctivitis. Because of its recurring character this is the most encountered cause of the recurrent type of this medical disorder, especially in the case of older patients. This condition is in most cases confused by patients with the dry eye disorder because of the gritty sensation that it can cause to the eyes. However, the lubricating treatment drops do not bring any improvement to this condition.Blepharospasm – SymptomsMost of the patients develop Blepharospasm without experiencing any warning symptoms. It can start with a gradual increase in blinking or with the development of an irritation of the eye. Some patients can also experience fatigue, emotional tension and sensitivity to a bright light. As the disease progresses to a much chronic stage, the symptoms become more often, and facial spasms may appear. Blepharospasm may decrease or cease while the patient is asleep or is concentrating on a specific action. Depending of which type of Blepharospasm the patient suffers from, he will probably also experience symptoms such as eye irritation, burning, tearing, foreign body sensation, uncontrollable tics or twitches of the eye muscles and surrounding facial area, crusty debris (that usually take place in the lashes or in the corner of the eyes and, especially on the lids), and also eye dryness, and red eyelid margins. In some cases, the patient can experience just the presence of dried tear secretions in the beginning of the day that can be sensed similar to small grains of sand. In these cases, the doctor will be capable to diagnose the disease after a careful and safe exam of the eyes and eyelids. The doctor usually uses a special magnifying instrument to perform the eye examination. This type of medical disorder generally affects only one eye but can spread easily to the other eye. Good hygiene of hands and face is very important in preventing the development of any type of the disease.Blepharospasm – TreatmentThe treatment for Blepharospasm is very difficult, but there are also some treatments that are based on medicines, while others imply surgery. The medicine therapy for this kind of disease is considered to be very difficult. There are some kinds of drugs which have different mechanisms of treating the disorder. They usually give short term benefits. One drug can be effective for some patients and not at all effective for others. When the action of one drug is stopped, in some cases, the replacement with another medicine brings long term benefits. There is no fixed treatment, nor a best treatment trial. Prescribing an effective treatment trial requires patience on the part of both the doctor and the patient. There are some drugs that are prescribed by the doctor in curing Blepharospasm and consist of Artane, or trihexyphenidyl, Cogentin, or benztropine, Valium, or diazepam, Klonapin or clonazepam, Lioresal or baclofen, Tegretol or carbamazepine, Sinemet and Modopar or levodopa. These medicines are not the only ones prescribed by the doctors and there are many other new treatments that are now recommended. The use of these drugs needs close monitoring from the doctor, and it is very important that the patient does not use another drug or stop the treatment without consulting the doctor. Before any surgical procedure is applied, patients are recommended to use safe, potentially effective, nonsurgical therapy such as botulinum toxin injections. Still functionally impaired patients suffering from Blepharospasm who have resisted or responded good to drugs or botulinum toxin are the first ones put on the list for surgical therapy. Now, protractor myectomy, which consists in the removal of some or all of the muscles responsible for eyelid closure, is considered to be the most effective surgical treatment for this kind of disease. Currents studies have found that myectomy has improved visual disability in almost all of the cases. There is a guide that is considered very common in the treatment of Blepharospasm, and is more challenging to be done by visually affected and frail patients and it requires almost perfect motor skills and, generally a mirror. There is a guide that is highly common in the treatment of Blepharospasm. The third phase consists in applying the antibiotic (usually erythromycin) and then pulling the lid away from the eye with the hand. The glands located on the lid margin are the meibomian glands, the zeiss glands, and the moll glands, and they are the target of the massage. Depending on the stage of the inflammation of the lid margin, there are various combinations of antibiotics or steroid drops and ointments which are prescribed to lead to an instant improvement. There are a lot of alternative treatments and can include biofeedback, acupuncture, hypnosis, chiropractic, or nutritional therapy. These types of therapies have not been yet approved. |
