Corneal Ulcer – General Information

The Corneal Ulcer, also known as ulcerative keratitis, is considered to be an inflammatory infective disorder of the cornea which consists in the destruction of the epithelial layer. In some cases, this disease can also affect the corneal stroma. There are several causes which can lead to the appearance of the disease. In most of the cases, the disease develops from the contact lenses, when they are not cleaned, handled or disinfected properly. This happens, due to the fact that contact lenses, especially the soft ones, have a very high level of water, and can easily contact bacteria or other infecting organisms. In the case of contact lens wearers, the most common cause of Corneal Ulcer is pseudomonas. The bacterial form of the disease increases the risk of some infective organisms that reproduce inside the cornea. This disorder can be similar to diseases that affect the corneal surface. There are cases in which this kind of disease appears from another condition which can include severe dry eyes, difficulty in blinking, or. This type of medical condition can appear from several causes such as herpes simplex viral infections, inflammatory conditions, corneal abrasions or damages, and many other systemic diseases. Patients with vitamin A deficiency are at an increased risk of developing the disease, and they can become blind in both eyes. The diagnosis of the disease is made by a medical observation using a magnifying view of slit lamp, which helps reveals the existing ulcer on the cornea surface.

Corneal Ulcer – Symptoms

In most of the cases, the Corneal Ulcer is considered a severe painful disease, because is directly exposed to the nerve, which can lead to other disorders such as tearing, squinting or even blinding of the eye. The signs and symptoms of this kind of disease can be similar to other eye conditions such as miosis, aqueous flare or redness of the eye. The most common signs and symptoms of the disease include red eye, severe pain, feeling that something is in your eye, tears, thick discharge draining from the eye, blurry vision, pain when looking at bright lights, swollen eyelids, a white round area on the surface of the cornea (which can be seen with the naked eye if the ulcer is in a late stage) and foreign body sensation. An eye that is affected by this disease becomes intensely sensitive, and this is why this type of condition generally produces intense pain. If the ulcer is located in a central area the vision gets impaired or can be completely absent. Tearing symptoms are present and the eye is red and especially hurts when you look at bright lights. The white area on the eye can be red and the patient can have a clear, yellow, or, in some cases, green fluid draining from it. The patient should go to a doctor if he experiences changes in the vision, severe pain, the feeling of something inside the eye, an obvious discharge coming from the eye or he has recently exposed himself to chemicals or flying particles.

Corneal Ulcer – Treatment

The treatment for Corneal Ulcer depends on the form of the disease and, most importantly, on the early diagnosis. To cure the infection caused by the bacterial form of Corneal Ulcer, the patient ought to undergo an intensive drug therapy with antibiotics. For the fungal form of the ulcer, the most common type of treatment is the one including topical anti fungal drugs. The viral form of the ulceration is provoked by the herpes virus and it is treated with antiviral medicines such as topical acyclovir ointment (that ought to be used for five to ten times each day). In combination with these medicines, doctors can prescribe another therapy which can include pain relievers. They can consist of topical cycloplegics prescribed as Atropine or Homatropine which dilate the patient’s pupil and stop the spasms that take place in the ciliary muscle. The superficial form of the ulcer can be healed in a few days with pain medications. Descemetoceles is also known as the deep form of the disease. It is generally treated with conjunctival grafts or conjunctival flaps, soft contact lenses or, in some particular cases, corneal transplant. In such cases, a therapy with vitamin C is very important.

Furthermore, in the case of patients who are suffering from Corneal Ulcer that has been caused by a vitamin A deficiency (Keratomalacia), a treatment trial with vitamin A is administered orally or intramuscularly. Doctors know that the use of topical corticosteroids or anesthetics is not recommended, because such medicines are contraindicated in this type of condition. A therapy with such medicines can cause infections with bacteria or other fungi, and in some cases, it can worsen the status of the disease. In all forms of this medical condition (Corneal Ulcer), Cycloplegic eye drops are the most effective type of treatment that can be applied, which, in all cases, rest the eye, and are used in combination with pain relievers. To know what type of antibiotic is needed to treat the Corneal Ulcer, loose epithelium and base are scraped off and examined in culture sensitivity studies. This helps doctors discover the organism that has caused the disease. The complete curing of the disease can last for more that four weeks, and in the case of the refractory form of the Corneal Ulcer, it can take more than one month to completely heal it. If the disease can not be healed with the available treatment options, or if the disease gets worse and can perforate the cornea, the doctor can make an emergency surgical procedure called corneal transplant. After the operation, if the patient does not need hospitalization, the doctor can prescribe topical eye drops which can be used at home. And if he experiences signs like pain, fever or discharge, he should contact the doctor again for further diagnosis and possible treatment, if it is needed.
