DermatophytosisDermatophytosis – General InformationDermatophytosis, also known as tinea pedis, ringworm of the foot and athlete's foot, is a highly common fungal infection which can affect persons of all ages. This type of infection can be taken from any public areas in which more than one person can enter and can include communal showers, fitness centers and locker rooms. Dermatophytosis affects the tissue surrounding the toes, but it can easily spread to the toenails or on the soles and sides of the patient’s feet. This kind of disease is known to respond well to the non-prescription medications that are recommended by the pharmacist, and can be applied directly on the skin. But there are cases in which the disease can become more severe and require aggressive medications. Dermatophytosis can be similar to many other fungal infections, all closely related through their name. This kind of condition can affect various parts of the patient’s body and can lead to the appearance of many other disorders which can include Tinea pedis which affects the feet, Tinea unguinum which affects the foot’s fingernails and toenails, Tinea corporis which can occur on the arms, legs, and trunk with ringworm, Tinea cruris, also known as jock itch and can affect the groin area, Tinea manuum which affect areas on the hands and palms, Tinea capitis, on the scalp, Tinea barbae affecting facial hair and, Tinea faciei, which is a face fungus, affecting the face area. The disease is more common in adult patients and is getting worse during the summer.Dermatophytosis – SymptomsDermatophytosis is caused by a group of fungal infections considered superficial infections (known as tinea). It generally affects the bearded skin of the face, scalp, groin, body, feet or nails. This group of superficial fungal infections is usually classified according to the location on the patient’s body. The signs and symptoms of this disease may be characterized by mild inflammations that could turn into severe vesicular reactions. When it affects the scalp (tinea capitis), this skin disorder produces some small papules on the skin of the head that spread very quickly and usually become inflamed. Tinea cruris (when it affects the groin) appears as raised, defined and red lesions that cover the buttocks, external genitalia and inner thighs. There are several signs and symptoms that are triggered by Dermatophytosis. Although they are part of a quite large list, change of a patient having all at a time is very little. These symptoms can appear as an itching, stinging and burning sensation between the toes, itching, stinging and burning on the soles area of the feet, itchy blisters, cracking and peeling skin, usually in the area between the toes and on the soles of the feet, excessive dryness of the skin on the bottoms or sides of the feet. However, there are cases in which the nails become thick, crumbly, ragged, discolored and pulling away from the nail bed. If the patient experiences one of the symptoms above, he should immediately contact the doctor, but there are cases in which Dermatophytosis can cause excessive redness, swelling, drainage or fever.Dermatophytosis – TreatmentThe treatment for Dermatophytosis depends on its severity. In most of the cases, the doctors recommend a treatment trial with several over the counter antifungal ointments, lotions, sprays and powders. In general, a therapy with these medicines does bring improvement. The most popular medicines include Terbinafine prescribed as Lamisil, Clotrimazole prescribed as Lotrimin and Miconazole prescribed as Micatin. Oral antifungal drugs are more effective because they go through the body to penetrate the affected area in the first days of starting the therapy. The oral antifungal drugs Terbinafine, Lamisil Tablets, and Itraconazole, or Sporanox Capsules have replaced older therapies, such as Griseofulvin, in the treatment of the disease. There are cases in which the Dermatophytosis disease has reached to a severe stage in which the topical treatment becomes inefficient. In such cases, the doctors can recommend an oral medication; the most common medical products used for this kind of treatment are Itraconazole prescribed as Sporanox, Fluconazole prescribe as Diflucan and Terbinafine prescribed as Lamisil. Fluconazole (or Diflucan) is prescribed as an antifungal agent and is used to treat the disease, but there are cases in which the medicine is taken in as an inhibitor. In most of the cases, the treatment uses a 5 mg dose per day, depending on the severity of the condition. This kind of medicine usually has the effect of increasing phenytoin concentrations, concentrations of theophylline, tolbutamide, glyburide, and gliclazide, effects of anticoagulants and cyclosporine concentration in the patient’s body. There are cases in which the treatment with Fluconazole is stopped without the doctor’s consent and the patient develops other disorders which can include clinical hepatitis, cholestasis and fulminant hepatic failure, diseases that can lead to death or many other deficiencies. Another treatment option applied in the case of patients with Dermatophytosis is considered Itraconazole or Sporanox which is usually administered in combination with Ketoconazole. This kind of fungal agent is administered in 500 to 600 mg doses per day. Doctors don not recommend Itraconazole to all the patients due to the fact that it can lead to the appearance of drug allergies and liver dysfunction. The main effect of the medicine is to increase tacrolimus and cyclosporine levels, plasma concentrations, digoxin levels and plasma levels of midazolam or triazolam in the patient’s body. The Sponarox medicine can be also administered in patients suffering from hepatic insufficiency. According to the scientists, the oral treatment with Itraconazole or the oral treatment with Terbinafine can cause, in rare cases, liver failure or even death. Because the oral treatment with Itraconazole used to treat Dermatophytosis and it can weaken the heart’s contractions, it must not be prescribed to patients who have a history of heart failure. If the Dermatophytosis disease is caused by a bacterial factor, the doctor can recommend a treatment with oral antibiotics. There are cases in which the doctor can prescribe wet dressings, steroid ointments, vinegar soaks and warm water compresses to relieve the pain and to clear up the soggy skin. Corticosteroids can also be used as treatment option for severe inflamed lesions, which can include the scalp infections. |
