Lactose Intolerance General Information

Lactose Intolerance is the inability to digest significant amounts of lactose, the main sugar in milk. This inability results from a lactase deficiency, the enzyme normally produced by cells lining the small intestine. Lactase transforms the milk sugar into simpler forms that can then be absorbed into the bloodstream. When the organism does not have enough lactase to digest the consumed lactose, it can be a very painful situation, although in most cases, it is not life threatening. Although not all people with lactase deficiency are bound to present symptoms, those who are considered to have Lactose Intolerance have various signs. The intolerance level varies with the severity of the lactase deficiency. Some intolerant people can drink a glass of milk without suffering from any symptoms, while others can not even tolerate a spoonful of milk in a cup of coffee. Some causes of Lactose Intolerance are well known. For instance, certain digestive diseases and small intestine injuries can reduce the amount of enzymes produced. In rare cases, the inability to produce lactase is present at birth. However, for most people, lactase deficiency is a condition that naturally appears over time.  After the age of 2, the body begins to produce less lactase. However, in many cases, the symptoms do not start to manifest until much later. Some ethnic or racial groups are more affected than others. For example, up to 75% of Americans of African descent and indigenous North American and 90% of Asian Americans are lactose intolerant. This assault is less common among people whose ancestors came from northern Europe.

Lactose Intolerance Symptoms

Common symptoms include nausea, cramps, bloating, gas and diarrhea; they appear within 30 minutes to 2 hours after the consumption of food or lactose containing beverages. The severity of symptoms varies depending on the amount of lactose each individual can tolerate. The tests most commonly used to measure the absorption of lactose in the digestive tract are actually tolerance tests. Other tests include the hydrogen breath test and the stool acidity test. The hydrogen breath test measures the amount of hydrogen in the breath. Normally, very little hydrogen is detectable in the breath. However, the non-digested lactose is fermented by the bacteria in the colon and various gases are produced, including hydrogen. Hydrogen is absorbed in the intestines, carried by the bloodstream to the lungs, and then expired. In this test, the patient takes a drink rich in lactose and the breath is analyzed at regular intervals. A high content of hydrogen in the breath indicates an imperfect digestion of lactose. Some foods, certain medications and cigarettes affect the accuracy of this test and should therefore be avoided before taking the exam. The test can be undergone by both children and adults. The lactose tolerance test and breath hydrogen test are not to be applied to infants or very young children suspected of suffering from Lactose Intolerance. Heavy lactose can be dangerous for very young children because they have a higher susceptibility to dehydration that can result from diarrhea caused by lactose. When an infant or young child manifests symptoms of Lactose Intolerance, many pediatricians simply recommend avoiding the consumption of milk. Some patients choose to include soymilk in their regular diet instead.

Lactose Intolerance Treatment

Lactose Intolerance is relatively easy to treat. There is no treatment to improve the body's ability to produce lactase, but symptoms can be controlled through diet. Young children with lactase deficiency should not eat foods containing lactose. Most children and adults do not need to avoid lactose completely, but patients differ in the amount of lactose they can handle. Some people may experience symptoms after drinking a small glass of milk, while other after drinking a much larger quantity. Others can eat ice cream and aged cheese, such as cheddar cheese or Swiss type without facing any problems, but no other dairy products. Mastery of Lactose Intolerance can be achieved through a diet based on the results obtained after running various tests on the amount of lactose that the person can tolerate. For those who severely react to very small amounts of lactose (who suffer from the most severe forms of Lactose Intolerance) or have trouble limiting their intake of foods containing lactose, it is possible to obtain lactase without a prescription.

One of the forms available is a liquid to be taken in along with milk. A few drops added to a liter of milk reduce the lactose content by 70% after 24 hours in the refrigerator. The response is faster if the milk is heated beforehand and a product containing double lactase liquid milk in which the lactose has been reduced by 90% is added. Now lactase is also available in chewable tablets to help digest solid foods that contain lactose. Three to six tablets are to be taken in just before a meal or snack. Milk and other products containing reduced lactose are available in many supermarkets. This milk contains all the nutrients of regular milk and remains fresh for about the same period or longer if it is pasteurized. If necessary, an acidity stool test, which measures the amount of acid in the stool, may be given to infants and young children. Non-fermented lactose is digested by the bacteria in the colon which produces lactic acid and other short chain fatty acids that can be detected in a stool sample. Moreover, poor absorption of lactose in the colon can result in the presence of glucose in the sample.

Although one could prevent Lactose Intolerance, it can not be healed. In order to mitigate or eliminate the symptoms, it is recommended to avoid dairy products containing a high proportion of lactose. In addition, lactose is acting as an inactive ingredient in 21% of prescription drugs and in 6% of medications. It is important to check with the pharmacist before taking a new drug. Milk and other dairy products are an important source of nutrients in the North American diet. The most important of these nutrients is calcium. Calcium is essential for growth. Starting at the end of the quarantine, a calcium deficiency may result in fragile and thin bones that could break easily (a condition called osteoporosis). Therefore, for children and adults who have Lactose Intolerance, the challenge is to get enough calcium into a diet where milk is almost absent.
