Osteosarcoma

Osteosarcoma General Information

Osteosarcoma is a type of cancer that affects the bones, being very common among children. The cells involved in the growing of bones are the osteoblasts so that this bone disorder develops from them. The teenagers who are experience a growth spurt are the most affected by this disease (80% of them) which is localized, in most of the cases, in the knee or close to the ends of the long bones. The part that is next to the knee of the thigh bone (distal femur) or lower leg (proximal tibia) can be affected by the cancer. Other part of the body predisposed to Osteosarcoma is the part of the bone belonging to the upper arm that is the most close to the shoulder, the proximal hummers. All of these locations of the cancer can be treated. But it can arise in other parts of the skeleton where the surgery may be harder to realize and process of curing too, for example, in the bones of the jaw, pelvis or shoulder. The causes are random and can not be predicted and usually come from errors of the DNA of cells involved in the process of intense bone growth. Environmental triggers that increase the risks of this cancer are believed to be the fluoride or radium from the drinking water. Osteosarcoma is also known as osteogenic sarcoma is a malignant type of cancer that in very few cases spreads to other parts of the body (metastasize) besides the bones. The spreading to lungs, other bones or internal organs is made through the bloodstream.

Osteosarcoma Symptoms

The symptoms depend almost all the time of the risk factors for the disease. Some factors of risk may be: being too tall for the age someone has, the presence of a non-cancerous (benign) disease affecting the bones, teenage growth spurts, the presence of very rare forms of cancer that are inherited (retinoblastoma, in children under five years old or Li-Fraumeni syndrome (predisposition to a big variety of cancers) and previous radiation treatments (mostly in small children) for another cancer. The symptoms or their appearance may differ from patient to patient because each person has another organism and health state. The size of the cancer and its location are factors very important in dealing with cancerous symptoms. Also, these signs are not specific for Osteosarcoma so that the disease may be easily confused with many other medical conditions (Ewing sarcoma).  For example, in young active persons, the disease is sometimes interpreted as excessive stress and trauma. The pain caused by Osteosarcoma is increasing with time becoming harder to suffer. The most common symptoms are pain and swelling or tenderness in the affected area. The usual symptoms that appear in most of the individuals are: limping, pain felt in the area of the affected bone that increases with each activity or lifting, slower movements of the limb affected and swellings near the affected location. The area that indicates the presence of affection tends to get red and to tender. But, a first alarming sign may be a fracture because any tumor weaknesses the bone and the area nearby. 

Osteosarcoma Treatment

Osteosarcoma does not benefit of an exact and efficient way of secure prevention, but an adequate examination and a well established treatment help most of the patients, especially small children to recover after this disease. You are strongly advised to see an orthopedist or a physician if you experience bone pain. But they rarely can diagnose a tumor of the bone so that the majority of the patients usually are misdiagnosed. They follow most of the time a simple physical therapy because the cancer is mistaken with muscle problems or cysts. To diagnose Osteosarcoma it is necessary to have an X-ray and then a combination of various scans such as PET scan, MRI, bone scan or CT scan. A complete examination usually finishes with a surgical biopsy that determines either the cancer is benign or malignant. An orthopedic oncologist should be the one who performs a biopsy because there is no cancer that demands a properly and well done procedure than Osteosarcoma. Otherwise, if it is mistaken, the patient could suffer an amputation of the affected limb.

Nowadays, the surgical resection is preceded by chemotherapy (also called neoadjuvant chemotherapy because it is given before any operation). The standard therapy is based on combination of high doses of ifosfamide and methotrexate with MTP (muramyl tri-peptide), leucovorin rescue, mesna, intra-arterial cisplatin caffeine, etoposide, adriamycin or BCD and combined limb salvage orthopedic surgery. Worrying is the fact this bone cancer has a very low survival rate in children although the chemotherapy is carefully followed. Very important during the treatment is the patient’s hydration so that a big quantity of fluids is given to him or her. Symptoms like vomiting and nausea are managed with Zofran or Kytril. The fight against anemia is also vital for the human organism so that Neulasta, Neupogen and epogen are prescribed to help in the production of blood’s white cells, especially neutrophils. The main aim for any treatment is to remove any tumors or cancerous cells from the body and then to proceed with chemotherapy that helps killing any remain dangerous cells that could develop tumors. The chemo is given intravenously or orally in most of the patients in order to remove from the body every cancerous cells, even those who are very small and do not appear on a regular check.
